We are currently preferred providers for the following private insurances:
- Blue Cross PPO
- Blue Shield PPO Non-Covered CA Plans
(Note – we can accept Blue Shield PPO Cover California Plans on an Out-of-Network basis) - Canopy through the John Muir Health System
- Covered California Insurance Plans PPO – Most Carriers
(Note – we can accept Blue Shield PPO Cover California Plans on an Out-of-Network basis) - Medicare Part B Standard and most Medicare Advantage Plans (No HMOS)
- Motor Vehicle Accident (Most Insurances)
- Tricare
- TriWest
- Workers’ Compensation (select carriers)
- Many other plans, call if you don’t see your plan listed.
We accept the following private insurances on an 'out-of-network' basis:
- Aetna PPO
- Humana PPO
- Blue Shield PPO Covered California Plans
(Blue Shield PPO standard plans are in-network and listed in the prior section) - Guardian/PHCS
- HealthNet and HealthNet Plus
- Multiplan/PHCS
- Private Healthcare Systems (PHCS)
- United Healthcare Medicare Advantage PPO Plans

If you are interested in services at our office and do not see your insurance listed above, please call or text us at (925) 284-6150 and our team would be happy to verify your coverage.
We are unable to bill HMO plans except some Canopy Health plans. We are able to see most HMO patients on a private pay basis.
There are some insurance plans we are not able to accept due to coverage areas, plan restrictions, etc. Our team can let you know if we are able to bill your insurance as well as provide information on our private pay rates in the event patients wish to be seen outside their insurance.
